Chapter 7 - The Breakthrough Scan

By 10:00 AM the next morning, Room 4 had been transformed into a high-intensity metabolic filtering suite. The large, blue continuous renal replacement therapy machine sat beside Thomas’s bed, its clear plastic rollers spinning steadily as they moved his blood through a high-flux dialyzer membrane, stripping out the potassium and toxic urea that had paralyzed his system.
His core temperature had been raised to a normal thirty-seven degrees. The change was small, but the physiological impact was immediate: his skin had lost its ghostly, cold gray hue, replacing it with a faint, warm pink tone that made Emma smile as she sat by his side.
I stood at the central console with Dr. Helena Vance (no relation to the attorney), the Chief of Neurology. She was a diminutive woman with grey hair cut into a sharp geometric bob, her eyes scanning Thomas’s charts with the speed of an optical character reader.
"The CT scan from yesterday was read incorrectly by the trauma resident," Vance said, her fingers clicking through the sagittal views of Thomas’s brain. "Look here at the junction of the pons and the midbrain. You see this dark area of low attenuation? That isn't cellular death. That’s focal vasogenic edema—trauma-induced swelling that is pressing directly against the descending corticospinal tracts."
"A functional compression," I said, my heart skipping a beat.
"Exactly," Vance said, turning to look through the glass at Thomas.
"It’s like a car parked on top of a garden hose. The water isn't flowing, but the hose isn't broken. His cortex is firing perfectly—that’s what your EEG showed. But the signals cannot pass through the swollen brainstem to reach his arms and legs. Except for these tiny micro-channels that control the intrinsic muscles of his hands. He’s using every ounce of his willpower to force a signal through a space no wider than a human hair."
She walked into the room, her reflex hammer in hand. She approached the bed, looking down at Thomas.
"Thomas," she said clearly. "My name is Dr. Vance. I am a neurologist. I want you to listen to my voice very carefully. We have cleaned your blood. The sedation is gone. I want you to try to open your eyes. Don't worry about your hands. Try your eyes."
Emma leaned over the guardrail, her breath catching in her throat.
For ten seconds, nothing happened. The room held its breath, the only sound being the rhythmic shirr-shirr of the dialysis rollers.
Then, Thomas’s left eyebrow twitched.
His eyelashes, thick and crusted with dried secretions from the crash, began to separate. With an agonizing, slow motion that looked like it required the lifting of a mountain, his left eyelid fluttered upward.
The eye was bloodshot, the pupil dilated but responsive to the light. He couldn't focus his gaze—his ocular muscles were still partially blocked by the brainstem pressure—but he moved his globe distinctly to the left, tracking the silhouette of his daughter.
"Dad," Emma whispered, her voice a tiny, trembling thing. "Dad, look at me."

Thomas’s eye shifted down, locking onto her small face. A single tear slipped from the corner of his eye, running down into the white adhesive tape on his cheek.
"He’s there," Dr. Vance said, her clinical detachment slipping for a fraction of a second as she reached up to adjust her glasses. "The brainstem edema is beginning to resolve under the osmotic pull of the dialysis. Dr. Parker, initiate a low-dose infusion of Hypertonic saline to accelerate the fluid shift out of his neural tissues. We are going to clear that garden hose."
