Chapter 8 - The Emergency on Highway 9

Two months later, mid-January brought the worst ice storm the county had seen in a decade.

A heavy, freezing rain had coated the roads in a thick layer of black ice, paralyzing traffic across the state. At 5:00 PM, the red emergency phone on Piper’s desk began to ring with frantic persistence.

Piper grabbed the receiver. "Dr. Vance."

"Chief, we have a multi-vehicle pileup on Highway 9 near the river bridge," the ER charge nurse reported, her voice laced with adrenaline. "Over twelve cars involved, including a school minibus. First responders are on scene, but the ice is delaying the county ambulances. We have multiple critical traumas incoming simultaneously. Dr. Sterling is requesting an immediate mass-casualty protocol call-in!"

"Initiate Protocol Gold," Piper ordered instantly, standing up and grabbing her trauma shears. "Clear Bays 1 through 8. Call in all off-duty surgical staff, get the blood bank to thaw thirty units of O-negative, and notify the OR suite to prepare four simultaneous rooms!"

Within ten minutes, the Saint Jude Trauma Wing transformed into an oiled machine of high-intensity precision.

Ambulance sirens howled through the freezing storm outside. The automatic double doors opened repeatedly, bringing in gantry after gantry of shivering, bloodied, and terrified victims.

Piper stood at the central triage desk, her high-visibility yellow trauma vest zipped over her scrubs, directing the flow of traffic like a general on a battlefield.

"Bay 2—tension pneumothorax! Get a chest tube in him now, Harrison!" Piper commanded. "Bay 4—pelvic fracture! Apply a pelvic binder and get a STAT pelvic X-ray! Bay 6—severe head injury! Call Neurosurgery!"

As the fifth ambulance backed into the bay, the lead paramedic jumped out, shouting over the noise of running engines.

"We've got an adult male, trapped under a crushed SUV frame for forty minutes! Severe crush injury to the left lower extremity, signs of hypovolemic shock, blood pressure sixty over forty! We had to execute a field extraction!"

The paramedics wheeled the stretcher through the doors.

Piper stepped forward to take over the primary assessment—and froze for a fraction of a second.

The man on the gantry, his face covered in soot, dried blood, and road grime, was her brother, Mark.

"Mark!" Piper breathed, her instincts flaring.

Mark’s eyes fluttered open, glassy with pain and blood loss. He looked up through the haze of shock, recognizing her voice instantly.

"Piper..." Mark gasped, his breathing shallow and rapid. "The ice... my truck skidded... the trailer hit me..."

"Dr. Harrison! Get over here now!" Piper shouted, her hands moving automatically to cut away Mark’s blood-soaked jeans.

The left leg was severely deformed, the femoral artery compressed under a massive hematoma. His skin was cold, clammy, and marble-pale—classic stage-three hemorrhagic shock.

"He's bleeding out from the deep femoral branch," Harrison declared, running the ultrasound wand over Mark's thigh. "We need to get him to the OR immediately or he's going to lose the leg—or his life!"

"I can't operate on him," Piper said, her mind racing through surgical protocols in a microsecond. "I am his sister. Hospital policy—"

"Dr. Vance!" Harrison interrupted, looking her dead in the eye over the chaotic noise of the trauma bay. "Dr. Sterling is currently in OR 2 with an aortic rupture! Dr. Miller is in OR 3 with a pediatric skull fracture! You are the only attending vascular surgeon on this floor who can repair a shredded femoral artery in under twenty minutes! If we wait for an on-call replacement, your brother will be dead in ten!"

Piper looked down at Mark. Her brother grabbed her wrist with his trembling, blood-stained fingers.

"Piper..." Mark whispered, tears cutting clean lines through the soot on his cheeks. "I trust you. I trust my life with you... please."

Piper closed her eyes for one long, deep second. She exhaled the personal fear, the family history, and the emotional noise. She opened her eyes, transformed entirely back into the cold, brilliant master surgeon.

"Prep OR 1," Piper commanded, her voice dropping into a register of iron authority. "Get six units of packed red blood cells, fire up the rapid infuser, and get me a vascular bypass tray! Now!"
