Chapter 9 - Blood and Iron

The operating room was an island of intense heat and brilliant light amidst the cold darkness of the winter storm outside.
"Tourniquet applied to the upper groin," Harrison called out, his hands working furiously beside Piper. "Suction!"
"Suction active," the tech responded.
Dark crimson blood surged from the torn tissue of Mark's upper thigh as Piper made a long, longitudinal incision over the femoral triangle.
"There's the rupture," Piper said, her fingers moving inside the blood-filled cavity with breathtaking speed. "The proximal artery is shredded. Clamp!"
Harrison handed her a vascular bulldog clamp. Piper placed it precisely above the tear, instantly halting the gush of blood.
"Blood pressure is rising! Seventy-four over fifty!" the anesthesiologist reported. "Keep the blood coming! Two units infused!"
"He has extensive arterial tissue loss," Harrison warned, looking at the ragged edges of the blood vessel. "We can't perform a primary anastomosis. There isn't enough length!"
"Then we harvest a saphenous vein graft from his right leg," Piper commanded smoothly. "Harrison, prepare the right leg. I will mobilize the proximal femoral stump. We have twelve minutes before the lower tissue suffers irreversible ischemia."
For forty-five minutes, the only sound in OR 1 was the rhythmic clicking of metal instruments, the hum of the suction machine, and the steady, urgent monitoring tones.
Piper worked with absolute, superhuman focus. Every stitch of the five-zero prolene suture was placed with sub-millimeter precision, reconstructing her brother’s shattered blood vessel like a master jeweler repairing a delicate gold chain.
"Releasing the clamp," Piper announced quietly.
She slowly unclamped the vessel.
Blood surged through the newly grafted vein. The pale, mottled skin of Mark's lower leg and foot instantly flushed with a healthy, warm pink color.
"Dorsalis pedis pulse is strong!" the anesthesiologist cheered from behind the screen. "Palpable pulse restored! We have full perfusion!"
Harrison let out a wild, breathless laugh. "You saved the leg, Dr. Vance! You saved the whole damn limb!"
Piper stepped back from the table, her hands covered in her brother's blood, her shoulders trembling slightly with residual adrenaline. She looked at the pulse monitor, confirming the steady, beautiful wave of life returning to her brother's body.
"Close the incision, Dr. Harrison," Piper said, her voice soft but filled with profound triumph. "Send him to the Surgical ICU. Tell his wife he's going to walk again."
She walked out into the scrub room, turning on the taps and letting the hot water wash away the blood from her hands and arms.
