Chapter 1 - The Sovereign Decree

The emergency room at Bellevue Hospital did not possess the quiet elegance of the Pierre Hotel ballroom, but it understood the exact same currency: life and death. The air smelled of sharp antiseptic, burnt copper, and the frantic, high-stakes sweat of trauma surgeons working beneath blinding halogen lights.

"Systolic is dropping to eighty! We're losing her!" a doctor shouted, his gloved hands deep in Sarah's side as the automated blood pressure monitor wailed a continuous, flat note.

"Push two more units of O-negative! Move!"

Outside the double steel doors of Trauma Room One, the entire hallway had been cleared of civilian traffic. Twenty men in identical tailored charcoal suits stood in a silent, impenetrable wall along the corridor, their hands folded neatly over their waistbands. At the center of the line stood Lorenzo Caruso.

He had not washed his hands. The dark, drying crimson of Sarah Miller’s blood covered his palms and stained the cuffs of his white dress shirt. Beside him, his six-year-old son, Leo, sat quietly on a plastic waiting room chair, wrapped securely in Lorenzo’s oversized black suit jacket, his small fingers clutching a plastic cup of water.

The elevator doors at the end of the hall chimed. Mr. Henderson, the floor manager from the Pierre Hotel, stepped out, flanked by two nervous-looking police detectives. Henderson’s face was slick with sweat, his collar unbuttoned as he scanned the line of armed mobsters.

"Mr. Caruso," Henderson stammered, his voice trembling as he approached the king of New York. "The... the hotel management is deeply sorry for the incident. We are conducting a full review of our security protocols. Regarding the girl, Sarah Miller... she was just a temporary hire. A nobody from Queens. We can handle her final expenses, ensure her family receives a modest settlement—"

Lorenzo didn't look up immediately. He slowly turned his head, his cold, abyssal gray eyes locking onto Henderson with a ferocity that made the floor manager instantly step back.

"A nobody?" Lorenzo's voice was a low, terrifying rumble that seemed to drop the temperature in the hallway by ten degrees. "That 'nobody' took three bullets meant for the heir to the Caruso family while your expensive security detail was cowering beneath the champagne tables."

"Mr. Caruso, please—"

"Silence," Lorenzo commanded, standing up to his full six-foot-three height. He walked over to Henderson until the manager could smell the gunpowder and expensive cologne radiating from him. "By tomorrow morning, Sarah Miller will no longer be an employee of your hotel. She will be my wife."

Henderson’s jaw dropped. The two detectives behind him exchanged panicked, wide-eyed glances. "Your... your wife? Mr. Caruso, she’s unconscious. She might not even survive the night."

"She will survive," Lorenzo whispered, his jaw flexing as he looked back toward the trauma room doors. "Because I have just given her the Caruso name. And nobody dies under my name without my permission. Find her brother, Toby. Bring him to my estate. If his insulin is late by even a single minute, I will buy your hotel just to burn it to the ground with you inside."
