Chapter 1 - The First Flame

The heavy silence hanging over St. Catherine’s Emergency Room was shattered by Marcus Reed’s guttural whimper. His forehead pressed hard against the cold concrete block of the wall, his expensive charcoal overcoat bunched in the tight, merciless grip of the stranger.

Naomi Vale watched, her chest heaving, her pulse hammering against her ribs like a trapped bird. She had spent twelve years working in this hospital, nursing every degree of human trauma, but she had never seen a man brought to his knees with such terrifying, silent efficiency. The blood soaking the left side of the stranger’s white shirt had broadened into a deep crimson stain, yet his hand remained steady, his posture unyielding.

"I asked a question," the dark-haired stranger murmured into Marcus’s ear, his tone deathly quiet. "Who is they?"

Marcus trembled, all his arrogant composure evaporating into thin air. "I... I don’t know what you’re talking about! It’s just me and Elise! Please, you’re breaking my wrist!"

"He’s lying," Elise whispered.

Naomi turned instantly toward the young woman, who was clutching her shattered wrist against her chest, her good eye wide with a mixture of terror and sudden, shocking hope. Tears carved clean lines through the dirt and dried blood on Elise’s cheeks.

"Elise, don't," Marcus grunted, twisting his head slightly, trying to glare at her. "Shut up!"

The stranger applied a fraction more pressure to Marcus's joint. A sharp pop echoed in the room, followed by Marcus’s agonizing shriek.

"Speak to her again, and I’ll sever the nerve," the stranger said without a trace of emotion. He looked over his shoulder at Naomi. His dark eyes were piercing, intelligent, and carrying the weight of a man who commanded empires from the shadows. "Nurse Vale. Your patient was about to tell us something."

Dr. Patterson stepped forward, his face pale, hands hovering nervously near his belt. "Look, whoever you are, security is on their way. We can't have vigilantism in my emergency department. Let him go!"

"Your security guards are currently having coffee in the basement, Doctor," the stranger replied calmly. "And if you had done your job, this nurse wouldn't have had to put her life on the line to protect a patient."

Dr. Patterson flushed with shame, faltering.

Naomi took a step closer to Elise, gently wrapping her arms around the trembling girl’s shoulders. "Elise, look at me. You are safe right now. He cannot hurt you in this room. What did Marcus mean when he said 'they'?"

Elise swallowed hard, her throat working painfully against the ugly purple fingerprints left on her flesh. "It’s... it’s not just Marcus," she sobbed. "He works for them. The Vanguard Syndicate. He... he brings them women. Girls from the suburban areas, girls who don't have powerful families to look for them. He tests us first. He breaks us to see if we'll stay quiet..."

Naomi felt a cold chill run down her spine. "A syndicate?"

"He has a list," Elise continued, her voice shaking violently. "A ledger in his apartment. Names of women, names of buyers, names of men in the city who pay to make things disappear. He told me tonight... he told me if I went to the police, he’d put my name on the auction block next week. He said nobody would ever find my body."

The stranger’s eyes narrowed. A dark, dangerous shadow passed over his features. "The Vanguard Syndicate. Dante Vance’s old territory."

Marcus went stiff. "How... how do you know that name?"

"Because Dante Vance owes me thirty million dollars, and his life," the stranger said softly. He shoved Marcus forward, releasing him so suddenly that the abuser collapsed onto the linoleum floor, gasping for air and holding his ruined wrist.

The stranger reached inside his coat, pulled out a thick leather wallet, and extracted a sleek, unembossed black card. He slid it across the nurse's station counter toward Naomi.

"My name is Lorenzo Rossi," he said simply.

Naomi’s breath caught in her throat. Even in the insulated, chaotic world of healthcare, everyone in the city knew that name. Lorenzo Rossi—the reclusive, formidable head of the Rossi estate. To the public, he was an untouchable shipping magnate and philanthropist. To the underworld, he was the Don—the undisputed king of the northern territories, a man governed by an ironclad personal honor code that kept the city's worst monsters at bay.

"You're bleeding out, Mr. Rossi," Naomi said, forcing her professional training to override her shock. She gestured toward his side. "You've lost at least half a liter of blood while holding that scoundrel against the wall."

A faint, wry smile touched Lorenzo’s lips. "A flesh wound. A minor disagreement with a competitor's lieutenant in an alley three blocks away. I walked in here to get stitched up quietly."

"Well, you're not leaving until I repair that side," Naomi said sternly, stepping over Marcus, who was groaning on the floor. She pointed at two beefy orderlies who had finally arrived at the doorway. "You two. Hold Marcus Reed right there. Call the police, charge him with domestic battery and terroristic threats, and do not let him leave this room."

"You can't do this!" Marcus yelled, his voice cracking. "Do you know who my boss is? You're all dead! Every single one of you!"

Lorenzo didn't even look back at Marcus as he allowed Naomi to guide him toward Bay Seven. "Your boss is about to have a very bad night, Mr. Reed. And as for you... Nurse Vale’s war has just begun, and I believe I’ve just enlisted."

Naomi looked at Lorenzo, her heart hammering. She saw the deadly determination in his eyes, but beneath it, an unmistakable promise of protection. She had started this fight to save one girl tonight, but as she prepped the suture tray, she realized the rabbit hole went far deeper than she could have ever imagined.
